What is Otoplasty?
Otoplasty, often referred to as ear pinning surgery, is a type of cosmetic procedure designed to alter the size, position, or shape of the ears. This procedure is commonly performed on individuals who have been born with overly large or protruding ears, or those who have been dissatisfied with previous ear surgery.It can also correct asymmetry, deformities from birth or injury, and overly large or misshapen ears. Â The goal of otoplasty is to improve the balance and proportion of the ears in relation to other facial features, enhancing the overall facial aesthetic.
Otoplasty, or ear pinning surgery, is unique among plastic surgery procedures as it can be performed on children as young as five, usually during their preschool years. Performing the surgery at a young age is to mitigate potential teasing or bullying once the child starts school. Prominent or protruding ears can have social, emotional, and academic impacts on a child. However, this procedure is not exclusive to children; adults are also candidates for otoplasty to modify the shape and contour of their external ears.
Ears can protrude due to various reasons. The antihelix, a small ridge in the ear, can cause the ear to stick out noticeably from the face. Similarly, conchal hypertrophy, where the concha, the area at the base of the ear, is enlarged and causes protrusion. Both these issues can be addressed through otoplasty. How is the Procedure Done?
Dr. Costeloe uses advanced techniques to reshape the cartilage through discreet incisions behind the ears, in the natural crease where the ear meets the head.This approach allows for precise modification while ensuring natural-looking results with minimal visible scarring.
During the operation, an incision is made behind the ear to reveal the ear cartilage. This cartilage is then repositioned, and parts may be removed if needed. Permanent sutures might be employed to secure the ear in its new position. After the adjustments are made, the skin at the back of the ear is stitched up, and a soft dressing is applied to protect the newly positioned ears. Your surgeon will remove the dressing a couple of days after the procedure and the sutures come out 7 days post-procedure.
What Are the Different Techniques?
Otoplasty can involve various techniques, including removing cartilage, reshaping the ear by creating or increasing folds, or repositioning the ear closer to the head.
Common techniques include:
- Cartilage scoring/suturing: Sculpting and using permanent sutures to hold the ear in its new position.
- Mustardé technique: Uses non-removable sutures to reshape the ear without removing cartilage.
- Furnas technique: Addresses overly prominent conchal bowl (deep part of the ear) by placing sutures to anchor it closer to the head.
- Incisional techniques: In cases of extreme prominence or deformity, part of the cartilage may be excised.
The specific technique will be customized based on your anatomy and aesthetic goals.
How Do I Prepare for the Procedure?
Before surgery:
- Stop smoking 3 weeks before and after.
- Discontinue blood-thinning medications or supplements (aspirin, NSAIDs, fish oil) as instructed.
- Wash your hair the night before surgery, as you won’t be able to for a few days post-op.
Prepare soft headbands and button-up clothing to avoid pulling anything over your ears.
What Is the Recovery Timeline After the Procedure?
Day 1–3:
Expect some swelling, mild pain, and bandaging around the ears. Discomfort is usually well-controlled with pain medication.
Week 1:
Most patients return to school or work. A lighter headband may be worn at night for protection.
Week 2–3:
Swelling continues to subside. Most normal activities can be resumed.
Week 4–6:
Final results become more apparent. Scar care may begin if advised.
